Thai drivers license

I want to get my Thai license. I have a US license for auto and motorcycle, and a international driving permit. I understand i also need

1) A medical certificate
2) Proof of address

Is there more?
The medical I imagine any clinic can do (the one that shares with a dentist?) but what proof of address?

The med cert you can get at the clinic next to the bakery - 70 baht.

For the proof of address you need to go to Dansingkhon and get a letter from immigration - 500 baht (you'll need the blue house book and ID card of the owner).

You'll have to go to the transport dept out on Petchkasem Rd, do a reaction test, colorblind test, watch a load of videos, do the multiple choice test (practice first as they're not obvious) and drive around a car park, takes a full day. I'm not sure if you can simply 'convert' your foreign license to a Thai one any longer.
